ELECTRONIC HEALTH RECORD ANALYST I - GRADE 11

Summary: Analyzes patient care information in the Electronic Health Record (EHR). Works collaboratively with ITD to resolve identified problems and make corrections to the EHR (Epic Care). Provides end-user training and develops end-user documentation in the HIM Section. Ensures EHR compliance with Joint Commission and all levels of regulatory guidelines. Makes recommendations as appropriate in regard to risk to the hospital and impact on patient care, patient safety, and physician and patient satisfaction.

Job Responsibilities: Serves as the initial point person of contact for EHR corrections for Providers and Administrators. Provides implementation and project management assistance through all phases of the development cycle. Investigates EHR errors and corrections, and provides appropriate feedback as necessary to related parties. Responsible for all final approvals to any deletion and/or corrections to EHR. Resolves issues. Performs regular random quality audits according to established quality plan. Other duties as assigned.

Education: Bachelor's Degree preferred in Health Information Management, or an Associate's Degree in Health Information Technology is required. Bachelor's Degree and experience in a related field may substitute for an HIM degree.

Experience: Two years experience in computer systems. Similar applications with healthcare experience preferred.

Licensure/Certification/Registration: Registered Health Information Administrator (RHIA) or Registered Health Information Technologist (RHIT) preferred.

Physical Requirements: Standing, walking, sitting, lifting, carrying up to 25 lbs. Requires normal or corrected vision and hearing and the ability to verbally communicate with employees and patients in person and on the telephone. Physical demands include office work and the mobility sufficient to attend meetings. The work environment is at a moderate noise level.

SR ELECTRONIC HEALTH RECORD ANALYST - GRADE 12

Summary: Provides leadership within the Team and interacts with all levels of management. Submits proposals for ongoing educational needs and leads the Team in peer review competencies. Assists in defining training requirements and conducts educational sessions and presentations for end users and team members. Provides guidance to and answers questions from staff on daily matters on case by case basis. Analyzes patient care information in the Electronic Health Record (EHR) and interacts with Providers, Administrators and other CCF personnel regarding EHR corrections and provides reliable tracking mechanics for changes to the EHR. Works with ITD to resolve identified problems and make corrections to the EHR, coordinates resolutions of patient amendment requests under HIPAA, prepares statistical reports for Medical Records Committee and other Administrative Committees. Provides end-user training and develops end -user documentation in Health Data Services. Provides implementation and project management assistance through all phases of the development cycle. Ensures EHR compliance with Joint Commission and all levels of regulatory guidelines. Makes recommendations as appropriate in regards to risk to the hospital and impact on patient care, patient safety, and physician and patient satisfaction.

Job Responsibilities: Leads team in conducting quality assurance reviews to improve the electronic record integrity. Identifies opportunities to conduct focused reviews and random sampling. Modifies quality tools as needed to meet the organizational needs and reports trends identified. Keeps abreast of the ever changing Epic environments and educates them to assist in maintaining their competencies. Conducts daily team meetings to cover day to day operational needs and provides management with weekly updates. Produces monthly reports collected from team submissions. Investigates errors and corrections and provides appropriate feedback to related parties. Researches and takes appropriate steps according to general guidelines. HIPPA, Joint Commission, federal, and state guidelines and CCHS policies and procedures. Other duties as assigned.

Education: Bachelor's Degree preferred in Health Information Management, or an Associate's Degree in Health Information Technology is required. Bachelor's degree and experience in a related field may substitute for an HIM degree.

Experience: Two years as an HIM Analyst with demonstrated progressive responsibility and a proven track record with strong analytical and communication skills or four years in a similar analytical support role. May substitute one year in the HIM Analyst role with a Bachelor Degree.

Licensure/Certification/Registration: Registered Health Information Administrator (RHIA) or Registered Health Information Technologist (RHIT) Certification required -- Bachelor's degree and experience in a related field may substitute for the certification requirement.

Physical Requirements: Physical demands require standing, walking, sitting, lifting, carrying up to 25 lbs. Requires normal or corrected vision and hearing and the ability to verbally communicate with employees and patients in person and on the telephone.
