THIS POSITION IS LOCATED IN SEVERANCE
Three to five years of nursing experience is required
Case Management experience with focus on Chronic disease, management, Med/Surg, Cardio ICU preferred.
Summary: Works collaboratively with medical home members to provide care and disease management to high risk patients identified in the ambulatory setting, focusing efforts on patient outreach and coordination of care for a panel of patients. Job Responsibilities: Conducts targeted outreach to defined panel of patients (DM, readmissions, etc) to ensure timely and efficient care delivery. Identifies barriers to receiving care and facilitates solutions. Performs comprehensive nursing assessments and/or reviews patient charts for the purpose of health promotion and prevention of illness for the defined panel of patients. Educates patients, staff, and medical team on behavior modifications, self-management and health literacy using a variety of educational methods and modes of communication. Works with practices on quality and process improvement initiatives. Assists in education, auditing quality, and workflow processes in ambulatory setting. Other duties as assigned.
Education: BSN preferred (Bachelor degree in a related field will be considered but not required)
Experience: Three to five years of nursing experience required. Licensure/Certification/Registration: Current state valid license to practice as a Registered Nurse (RN).
Physical Requirements: Requires full range of motion,manual and finger dexterity and eye-hand coordination. Requires corrected hearing and vision to normal range. May require some exposure to communicable diseases or bodily fluids. Medium Work-Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. Physical Demands requirements are in excess of those for Light Work.
