Summary: Oversees and implements the Case Management program. Manages the Case Management Department; discharge planning, utilization management and denials management. Collaborates with the medical staff, Directors and Executive Management to provide support and analysis to achieve hospital goals and strategic plans. Job Responsibilities: Plans, directs and supervises the Case Management program. Manages operational budget. Facilitates growth and development of the case management department consistent with Cleveland Clinic's philosophy. Manages staff and handles all personnel management responsibilities; hiring, compensation, performance management, development and training. Facilitates communication and collaboration between care providers (i.e. nursing, physicians, community resources, insurance companies). Uses data for performance improvement strategies for case management. Provides for a systematic process to collect and provide data. Maintains currency with best practices and technological advances. Other duties as assigned.

Education: Graduate from an accredited school of nursing, BSN preferred. CCM or alternate certification (CPUR, CPHQ) preferred.

Experience: Minimum five years in utilization management, case management or discharge planning. Three to five years in hospital-based nursing. Previous management experience required preferably in the current field of Case Management.

Licensure/Certification/Registration: Current state RN license. Certification in CPHQ or Case Management preferred. Physical Requirements: Light Work - Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly (Constantly: activity or condition exists 2/3 or more of the time) to move objects. Even though the weight lifted may be only a negligible amount, a job should be rated Light Work: (1) when it requires walking or standing to a significant degree; or (2) when it requires sitting most of the time but entails pushing and/or pulling of arm or leg controls; and/or (3) when the job requires working at a production rate pace entailing the constant pushing and/or pulling of materials even though the weight of those materials is negligible. NOTE: The constant stress and strain of maintaining a production rate pace, especially in an industrial setting, can be and is physically demanding of a worker even though the amount of force exerted is negligible.
