Summary: Evaluates and conducts medically prescribed physical therapy treatment programs.
Job Responsibilities: Performs initial and on-going assessments of patient's condition. Establishes, revises and evaluates a plan of care which is appropriate to problems identified and involves the patient/family according to their capabilities and desires. Performs therapy interventions utilizing standard physical therapy techniques and skills as appropriate for the age and condition of the patient. Supervises activities of physical therapy assistants, students and other support personnel. Other duties as assigned.
Education: Graduate of an accredited Physical Therapy program. Must be able to read, write, and communicate with patients.
Experience: Minimum one (1) year clinical experience as a licensed physical therapist in an acute, rehab or skilled nursing setting.
Licensure/Certification/Registration: State licensure as a Registered Physical Therapist. CPR Certification. Valid Ohio Driver’s license and current auto insurance $100,000/$300,000 coverage.
Physical Requirements: Must be able to climb stairs, pull, push and facilitate movement of the patient. Must regularly lift and/or move up to 25 pounds, frequently lift and/or move up to 50 pounds and occasionally lift and/or move more than 100 pounds. Must be able to transport laptop computer and/or necessary medical/patient equipment and supplies. Must have normal or correctable vision. Must have normal or correctable hearing to be able to hear normal conversations.
