Summary: With indirect supervision of a LISW, coordinates care and services of select patients across the continuum of illness. Promotes effective utilization of health care resources to achieve optimal clinical and resource outcomes. Conducts needs assessments and provides information and referral with community resources.
Job Responsibilities: Assesses psychosocial risk factors. Evaluates of prior functioning levels, support systems, reaction to illness and ability to cope. Actively manages risk; identifies potential adverse outcomes and works toward reducing or eliminating risks. Assesses patient and family financial, social, emotional, educational and environmental resources to identify needs based on current medical condition. Develops individual plan of care based on findings. Uses social work training in growth and human development, family dynamics, cultural and religious diversity to provide interventions. Participates in discharge planning for complex patients. Assists Care Managers with discharge planning. Screens and coordinates all new SNF and Rehab referrals. Other duties as assigned.
Education: Social Worker with a Master's Degree required. Knowledge of NASW Code of Ethic practices.
Experience: Hospital Social Work strongly desired. Exposure and/or experience in pre-acute and post-acute care; knowledge of available community resources preferred.
Licensure/Certification/Registration: Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC) in the State of Ohio. Social Worker or Case Management Certification within one year of eligibility.
Physical Requirements: Requires direct patient contact. Requires frequent walking, telephone use, computer use, bending, reaching and lifting items up to 25 pounds. Requires working well under pressure and professional demeanor under adverse conditions. Extensive reading, writing and use of office equipment. Normal or corrected hearing and speech are required to communicate with patients, family, outside agencies and hospital personnel. Good visual acuity through normal or corrected vision.
