GENERAL SUMMARY: Provides medical social services to assigned patients including assessment and intervention in accordance with agency policies and procedures. Maintains timely communications with others involved in the care of the patient. Assists primary nurses and therapists in problem resolution and facilitates patient/caregiver referrals to appropriate community resources. Participates as a member of a patient-focused interdisciplinary care team.
MINIMUM QUALIFICATIONS:
Education, Knowledge, Skills and Abilities
Graduate of an accredited school of social work; Master's degree
Requires knowledge of psycho-social assessment and techniques.
Thorough knowledge of how to access community resources, how to interpret the impact of legislative and health policy changes on the individual's access to care.
Must be able to travel to patient's home.
Must be able to read, write and communicate with patients.
Must be able to work in settings with infectious diseases.
Must present a professional appearance at all times.
Required Length and Type of Experience
Two years clinical experience as a licensed medical social worker in an acute setting.
Experience and competence in multidisciplinary discharge planning.
Required Licensure, Certification or Registry
Current Ohio licensure as a licensed independent social worker (LISW)
Current CPR certification
Valid Ohio Driver's license.
Current auto insurance $100,000/$300,000 coverage.
