Summary: Supports the physician and interdisciplinary team in facilitating patient care, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction while managing the cost of care and providing timely and accurate information to payors. Integrates and coordinates utilization management, care facilitation and discharge planning functions.
Job Responsibilities: Completes Utilization Management and Quality Screening for assigned patients. Applies approved clinical appropriateness criteria to monitor appropriateness of admissions and continued stays, documenting findings based upon Department standards. Identifies at-risk populations using approved screening tool and follows established reporting procedures. Monitors LOS and ancillary resource use, intervening as appropriate. Coordinates/facilitates patient care progression throughout the continuum. Works collaboratively and maintains active communication in a team setting with physicians, nursing and other members of the multi-disciplinary care team to effect timely, appropriate patient management. Addresses/resolves system problems impeding diagnostic or treatment progress. Manages all aspects of discharge planning for assigned patients. Ensures that all elements critical to the plan of care have been communicated to the patient/family and members of the healthcare team and are documented as necessary to assure continuity of care. Meets directly with patient/family to assess needs to develop an individualized continuing care plan in collaboration with physician. Meets Core Case Management competencies, including adhering to regulations. Stays current with regulations which impact the care of patients and the Clinic's regulatory compliance. Promotes individual professional growth and development and meets mandatory/continuing education, skills competency, supports department-based goals which contribute to the success of the organization; serves as a preceptor, mentor, and resource to less experienced staff. Represents the department on committees. Other duties as assigned.
Education: Registered Nurse; Bachelor's degree in Nursing or related field preferred.
Experience: Three to five years recent acute care nursing experience required. Excellent communication and negotiation skills necessary to work collaboratively with multidisciplinary teams for optimal outcomes. Demonstrated ability to work independently as well as with teams. Must be flexible, open-minded and adaptable to change.
Licensure/Certification/Registration: Current and valid license to practice as a Registered Nurse in the state of Ohio. Professional certification as a Case Manager required within 12 months of eligibility. Valid certification options available on Care Management sharepoint site.
Physical Requirements: Requires direct patient contact. Requires extensive walking, telephone use, computer use, bending, reaching and lifting items weighing 20 pounds or less. Must be able to work well under pressure and maintain professional demeanor under potentially adverse conditions. Extensive reading of medical terminology, writing and use of multiple business machines. Good visual and hearing acuity through normal corrected mechanism.
